GENERAL LIABILITY RELEASE

Event: Onsite Activity for Purpose Bible Institute (PBI)

By entering my full legal name below and submitting this form, I acknowledge that I have read, understand, and voluntarily agree to the terms of this Release of Liability.

I hereby release, indemnify, and hold harmless Greater Antioch Missionary Baptist Church (GAMBC), Purpose Bible Institute (PBI), and their respective employees, representatives, agents, volunteers, affiliates, and related organizations from liability for property loss or damage, personal injury, or death arising from or related to my participation in onsite activities associated with Purpose Bible Institute (PBI), to the extent permitted by law.

I further agree to follow all applicable rules, instructions, policies, and safety guidelines issued by GAMBC, PBI, their representatives, affiliated organizations, and/or participating vendors while taking part in these activities.

PHOTO/VIDEO RELEASE

I hereby grant Greater Antioch Missionary Baptist Church (GAMBC) / Purpose Bible Institute (PBI) ("the Organization") permission to record, photograph, film, and/or capture my image, voice, and likeness in any media format, including but not limited to:

Photographs
Video recordings
Audio recordings
Live or recorded digital media

2. Rights Granted

I authorize the Organization to use, reproduce, publish, distribute, display, and create derivative works from these materials for purposes including:

Marketing and promotional materials
Social media content
Websites and digital platforms
Educational or informational materials
Internal or external communications

This permission is granted without compensation and applies worldwide, in perpetuity.

3. Waiver

I waive any right to inspect or approve the finished product or any use of my likeness. I release the Organization from any claims related to the use of these materials, including but not limited to invasion of privacy, defamation, or misrepresentation.

4. Ownership

All photographs, recordings, and related media captured by the Organization become the exclusive property of the Organization.

5. Consent for Minors (if applicable) If the participant is under eighteen (18) years of age, a parent or legal guardian must provide consent on behalf of the minor participant.
